PLAYER REGISTRATION FORM

The registration fee includes the following: match day kit, match day pitch hire, match day referee fees (when available), x2 weekly team training session (term time only) and a technical session that will run throughout professional coaching staff (FA level 2 or UEFA certified), hire of 3G training facilities, training equipment, FA & League player registration fees, team affiliation fees, public liability insurance cover, personal injury cover, training facilities cover. Once your child is registered, DNA will not be able to offer refunds under any circumstances.
